HIPAA Notice of Privacy Practices

Our Commitment to Your Privacy
Second Set Health, Inc. is committed to maintaining the privacy of your protected health information (PHI) in accordance with the Health Insurance Portability and Accountability Act (HIPAA). This notice describes how medical information about you may be used and disclosed and how you can get access to this information. It should be read together with our Privacy Policy.
Uses and Disclosures
We may use or disclose your PHI for treatment, payment, and health care operations. Treatment includes providing, coordinating, or managing your health care. Payment includes billing and collection. Operations include quality assessment and improvement activities, customer support, security monitoring, and service improvement. Health care operations may also include using qualified service providers (business associates) who assist us with hosting, communications, payments, and optional automated processing such as speech-to-text or summarization, subject to HIPAA requirements where applicable.
Your Rights
You have the right to: (1) request restrictions on certain uses and disclosures of your PHI; (2) receive confidential communications; (3) access and obtain a copy of your PHI; (4) request amendments to your PHI; (5) receive an accounting of certain disclosures; and (6) obtain a paper copy of this notice.
Our Duties
Second Set is required by law to maintain the privacy of your PHI, provide you with notice of our legal duties and privacy practices, and abide by the terms of this notice. We reserve the right to change the terms of this notice at any time, and the new notice will be effective for all PHI that we maintain.
Security Safeguards
We implement administrative, physical, and technical safeguards to protect PHI. Sensitive data is designed to be encrypted in transit (TLS 1.2+) and at rest across supported production systems. Access to PHI is restricted to authorized personnel on a need-to-know basis.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us at support@secondset.health or with the Secretary of the U.S. Department of Health and Human Services. We will not retaliate against you for filing a complaint.
